Winter Pressures (in partnership with Age Connect Wales) is available to people coming out of hospital, providing them with 6 weeks of care and support.
A care plan will be created following our first contact, and will include our services as well as signposting to other organisations. Regular welfare checks by telephone will be included throughout the 6 week period.
